Please complete your prescription request slip and hand this into the Pharmacy or post into our prescription box outside on the ground floor.
If you are housebound or would experience difficulties doing this, please telephone the practice BETWEEN 11AM AND 4PM.
If you wish to register for online prescription requests, please telephone reception to request a registration form. You will be unable to follow the link below until you have registered via the practice.
